Below are links to the forms which financially-sponsored Students and Chaperones need to complete and bring with them when the SCVSEFA-chartered bus departs San Jose for the trip to CSEF on April 29th. Sponsored students include:

- middle school Isabelle Stone Best Biological Science Project Award winners
- middle school Castro Family Best Physical Science Project Award winners
In addition, non-sponsored students travelling in the SCVSEFA-contracted bus must also complete the “RISK” form below.
